We can help people in this situation to regain their abilities so they can keep their independence in their own home for as long as possible.
We can provide a short-term package of home care that rebuilds the skills you need to live independently, reducing your need for ongoing support.
If you are recovering from illness or injury or you've lost some of your normal abilities because you've been away for a while, you may benefit from this service.

You can request a care assessment for yourself or for someone you know.
Who can get help?
We can provide support to people facing substantial or critical risks to their independence or wellbeing. For those with moderate or low risks we can put you in touch with other organisations that might be able to help. Read more about who can get help with care.
