Carers Assessments
If you look after a relative or friend who cannot manage on his or her own you are a carer. The impact of caring is varied ranging from practical and emotional stresses to relationships and the impact on your work.
Support is available for you as a carer to help you continue your caring role. To find out what support you may need and what is available to help we need to discuss what is important to you.
Your needs can be identified in a number of ways: -
- A carer’s assessment – identifying your needs as a carer;
- A Joint Assessment – identifying your needs as a carer as part of the discussion of the needs of the person you care for;
- A carers self assessment – An easy-to-complete form that you return after identifying your needs, which will then be discussed by yourself and a Care Manager/Care Co-ordinator;
Carers can have the support of a friend or advocate to help them through the “assessment process”.
The person you care for does not need to know about your assessment if you do not want them to know.
