Assessing your needs
If you have a physical disability or have difficulty
with basic household tasks, the service may be able
to provide some assistance.
The first stage in this process will be assessing your
needs. A member of staff from the Social Care and Health
Department
or the Health Care Trust (usually an Occupational Therapist
or District Nurse) will meet with you to find out which
tasks you
have difficulty with in your home. This information
will be used to determine firstly, whether you need assistance
to live safely
in your own home and secondly, if you do, what kind
of
assistance you require. To arrange an assessment please
telephone Adult
Services Enquiries on 0845 130 4959. Or ask your GP
or District Nurse if you have one.
