What does a Statement of Special Educational Needs look like?

Timetable for assessment:

The time taken from a request for Statutory Assessment to the issuing of a Statement of SEN should not exceed 26 weeks except in prescribed circumstances.

The statement of special educational needs is set out in the following way:

 

Part 1 - Introduction

  • Child's name, address & date of birth
  • Childs home language and religion.
  • Names and address (es) of child’s parents

Part 2 - Special Educational Needs

  • Details of each and every one of the child’s special educational needs as identified by the LEA in the Statutory Assessment
  • Details of the advice received and attached as appendices

Part 3 - Special Educational Provision

  • Objectives that the special educational provision should aim to meet.
  • Provision to meet the needs set out in Part 2 and to meet the objectives
  • Monitoring arrangements, including short-term targets and procedures for reviewing progress.

Part 4 - Placement

  • The type and name of the school where special educational provision set out in Part 3 is to be made.
  • Arrangements for education otherwise.

Part 5 - Non-Educational Needs

  • Any non-educational needs for which the LEA considers appropriate if the child is to benefit from provision set out in Part 3.

Part 6 - Non-Educational Provision

  • Provision required to meet non-educational needs set out in Part 5, as agreed with health and/or social services and the LEA.
